Pines offers care coordination services free-of-charge to all Pines patients. Pines Care Managers are a group Registered Nurses and Social Workers who serve as advocates for our patients. They help patients and their families sort through options and resources. These caring individuals often coordinate multiple services to ensure each patient’s needs are met.
Pines Care Coordinators work with patients, their families, and medical team members to coordinate all aspects of patient care. They collaborate with all providers involved in a patient’s treatment to ensure the patient receives comprehensive care.

Education on medication and assistance with medication pill box refills
Providing education about your disease or illness
Listening to your concerns
Monitoring your progress
Serving as a liaison between pediatric patients, teachers and schools
Contacting loved ones at your request
Making referrals to other agencies as needed
Communicating with your insurance company at your request
Guidance for long term care services: at home, nursing home and boarding home
Assistance with living wills and information about power of attorney
Support and assistance throughout open child protection assessments and DHHS reunification process
