Health Insurance
Employees (20 or more hours) are eligible for health insurance coverage. Health coverage will be effective on the first day of the month following the employee's start date. Pines offers three health plans. A portion of the premium is paid by the employee.

Retirement Options
All employees are eligible to participate in retirement options. Pines Health Services has chosen Lincoln Financial Group to provide investment options and administrative services for our 403(b) retirement plan. You may enroll in this plan immediately upon your date of hire.

Life Insurance
All full-time employees (35 or more hours) are eligible for life insurance. Pines Health Services provides Basic Life and Accidental Death and Dismemberment (AD&D) insurance to employees in an amount equal their annual earnings to a maximum of $100,000.00. This coverage is provided through
The Standard.
If you would like additional life insurance to meet your financial needs, you can purchase voluntary life insurance through after-tax payroll deductions. Coverage elected will be in increments of $10,000.00 to a maximum of $200,000.00. Click here for more details about additional life insurance.
Dependent Life Insurance
Dependent life insurance is also available. This coverage provides a spousal benefit of $10,000.00 and $6,000.00 for dependent children. The cost of this benefit is $1.09 per pay period.
Short Term Disability (STD) Insurance
All full-time employees (35 or more hours) are eligible for Short Term Disability Insurance. Pines Health Services offers Short Term Disability through The Standard. Employees are responsible for the premium if coverage is elected. The cost of this benefit is based on your annual salary. The short-term disability plan pays a benefit for each week you are unable to work because of injury or illness. Your weekly benefit is 60 percent of your pre-disability earnings reduced by deductible income. Important contract limitations apply.

Long Term Disability (LTD) Insurance
All full-time employees (35 or more hours) are eligible for Long Term Disability Insurance. Pines Health Services offers Long Term Disability through The Standard. The long term disability plan pays a benefit for each month you are unable to work due to a disabling condition. Enrollment is automatic and Pines pays the full cost for your coverage. Your monthly benefit is 60 percent of your pre-disability earnings reduced by deductible income. Benefits begin on your 91st day of disability and continue until age 65 if you are unable to return to work. Important contract limitations apply.

Earned Benefit Time
Pines Health Services participates in an Earned Benefit Time (EBT) program for vacation, personal, and sick time. This means that for every hour worked; you may accrue a portion of that time as EBT. The basic annual allowance for a regular full-time employee is 12 days for the first year. Upon completion of a 90-day orientation period, employees may use their accrued time as s/he chooses, providing that staffing needs have been adequately met. Employees have the ability to carry over EBT from year to year up to a maximum accrual of 36 days or 288 hours. At that point, EBT stops accruing, as it is important for our employee to periodically spend time away from their job.

Providers' EBT is based on contract.
FSA
Pines Health Services offers access to participate in a Flexible Spending Account (FSA). This plan allows you to make contributions towards your benefits with pre-tax dollars. These dollars are not subject to FICA, federal or state income taxes. With an FSA, you elect to have your annual contribution deducted from your paycheck in equal installments throughout the year. The amount of your pay that goes into an FSA will not count as taxable income, so by participating, you are actually increasing your take home pay! FSA dollars can be used during the plan year to pay for qualified expenses and services. A Medical FSA allows reimbursement of qualifying out-of-pocket medical expenses. A Dependent Care FSA allows reimbursement of dependent care expenses, such as daycare, incurred by eligible dependents.
Employee Assistance Program (EAP)
All employees are eligible for our Employee Assistance Program (EAP). EAP offers support, guidance and resources that can help you resolve personal issues and meet life's challenges. This service is provided at no cost to you by Pines.
Corporate Discount Program
All employees are eligible to take advantage of our Corporate Discount Program. Pines employees receive discounts at a number of businesses in the area.
