Transferring to Pines

At Pines we make it easy to transfer to a new provider within our organization. If you are established with another health care organization but looking to join Pines and begin receiving high quality health care from our team, we will assist you in obtaining your medical records from your current health care provider.

Current Pines Patients

If the Women’s and Children’s Health Center will be closer to home, or more convenient for you just let one of our medical office specialists know and they’ll help you get settled in with your new health care provider.

New Patients

Switching from one provider organization to another can be a challenge, but not at Pines Health Services. Our medical office specialists will be with you every step of the way.

Here is what happens:

  1. Fill out a Medical Record Transfer Form available at any Pines Health Center
  2. Relax, we’ll request your health information and transfer it into our system
  3. We’ll call to schedule your first appointment with your new Pines provider once your health information arrives.
