Description:
Pines Health Services has an immediate opening for a full-time RN at their Surgical Services office located on the campus of Cary Medical Center. Qualified applicants must demonstrate a commitment to the vision, mission, and goals of PHS, modeling the values and culture while delivering exceptional patient care. Qualified applicants will be responsible for supporting surgeons in the office to include triaging patients, providing patient education, entering patient data into the electronic medical record, screening patient calls to the providers and assisting providers with procedures. The applicant will also be responsible for coordinating surgical procedures; this includes developing functional relationships with other individuals and departments, including but not limited to One Day Surgery, Operating Room, Rehabilitation and nursing home facilities, and other medical providers. Applicant must be competent with computers, have excellent verbal and written communication skills, be an outstanding listener, be a problem solver, prioritize workload, and be able to work professionally and independently.

Care Manager is responsible for providing care coordination and health education for Primary Care patients, to include low risk, at risk, and socially high risk patients who receive medical care from Pines Health Services. Coordinates and monitors the assessment, treatment planning process and follow up for each patient, serves as an advocate and liaison for and on behalf of patients. Maine State Registered Nurses License required.
