Position Title: Care Coordinator

Caribou Health Center

M - F 8:00 am - 5:00 pm;May require evening hours depending on patient needs.

Pines Health Services has immediate openings for Care Coordinators in both clinical and social work areas. Positions will be responsible for patients in the Caribou, Presque Isle and St. John Valley area. Qualified applicants must demonstrate a commitment to the vision, mission, and goals of PHS, modeling the values and culture while delivering exceptional patient care. Care Coordinators will be responsible for providing care coordination and health education for Primary Care patients, to include low risk, at risk, and socially high-risk patients who receive medical care from Pines Health Services. Coordinates and monitors the assessment, treatment planning process and follow up for each patient, serves as an advocate and liaison for and on behalf of patients. Qualified applicants will have a Bachelor of Social Work or related field and or Maine State Registered Nurses license.
